Kane County Community Health Assessment

Kane Health Counts, a collaborative including the Kane County Health Department and local hospitals, is collecting data for a Community Health Assessment. This is an important process that helps the county and its healthcare partners prioritize resources for health needs.

We are facilitating a one-hour, online focus group about Behavioral Health needs and we would like you to attend.
